Senso de coerência, resposta imunológica e presença de doenças bucais oportunistas em paciente com vírus HIV

Abstract

Introdução: A teoria salutogênica foi desenvolvida por Antonovsky em 1979 e vem merecendo destaque no contexto de saúde, tendo o Senso de Coerência (SOC) como seu elemento central. O SOC se relaciona ao bem-estar mental na tentativa de explicar o porquê de algumas pessoas permanecerem saudáveis frente ao estresse e outras adoecerem. Sendo assim, muitos estudos relacionam o SOC a condições adversas de saúde. A Síndrome da Imunodeficiência Adquirida (AIDS) compromete o sistema imunológico de maneira sistêmica e favorece lesões oportunistas, o que pode levar o indivíduo ao óbito, sendo uma doença ainda muito estigmatizada. Objetivo: Verificar a possível relação entre Senso de Coerência, resposta imunológica ao tratamento com antirretrovirais e presença de doenças bucais oportunistas em pacientes com vírus HIV. Metodologia: Estudo quali-quantitativo e transversal desenvolvido com 144 pacientes de ambos os gêneros portadores do vírus HIV e em acompanhamento no Hospital Dia Prof.ª Esterina Corsini em Campo Grande (MS). Os pacientes passaram por exame clínico bucal, coleta sanguínea para avaliação do estado imunológico e responderam o questionário do SOC. Resultados: Dos 144 participantes, 14,6% deles (n=21) apresentavam alguma doença bucal oportunista. A média do escore SOC foi de 49,70 pontos e não variou em pacientes com e sem doenças bucais. A média de CD4 foi de 585,17 por ml e se mostrou menor em pacientes com doenças bucais, sendo que o mesmo não ocorreu com a taxa de carga viral. Conclusão: Não houve relação entre o SOC e a presença de doenças bucais oportunistas e nem entre o SOC e a resposta imunológica dos pacientes. Nos pacientes com doenças bucais oportunistas, a taxa de CD4 mostrou-se significativamente menor do que nos pacientes sem doenças bucais oportunistas.

ABSTRACT - Introduction: The salutogenic theory was developed by Antonovsky in 1979 and has been highlighted in the context of health, with the Sense of Coherence (SOC) as its central element. SOC relates to mental well-being in an attempt to explain why some people remain healthy in the face of stress and others become ill. Thus, many studies relate SOC to adverse health conditions. The Acquired Immunodeficiency Syndrome (AIDS) compromises the immune system in a systemic way and favors opportunistic lesions, which can lead to death, being a disease still very stigmatized. Objective: to verify the possible relationship between Senso of Coherence, immunological response to treatment with antiretrovirals and presence of opportunistic oral diseases in patients with HIV. Methods: A quali-quantitative and cross-sectional study developed with 144 patients of both genders carrying the HIV virus and being followed up at the Dia Prof.ª Esterina Corsini Hospital in Campo Grande (MS). The patients were submitted to oral clinical examination, blood collection to assess the immune status and answered the SOC questionnaire. Results: Of the 144 participants, 14.6% (n = 21) had some opportunistic mouth disease. The mean SOC score was 49.70 points and did not vary in patients with and without oral diseases. The mean CD4 count was 585.17 per ml and it was lower in patients with oral diseases, and this was not the case with the viral load rate. Conclusion: There was no relationship between SOC and the presence of opportunistic oral diseases and neither between the SOC and the immunological response of the patients. In patients with opportunistic oral diseases, the CD4 ratio was significantly lower than in patients without opportunistic oral diseases.
