FATORES DE RISCO E ASSISTÊNCIA DE ENFERMAGEM AO PACIENTE COM AVC: UMA REVISÃO INTEGRATIVA

Abstract

This study aims to analyze the main risk factors and the organization of nursing care for patients affected by Stroke in the home environment, after hospital discharge. Stroke represents one of the leading causes of disability, requiring prolonged care and effective care planning. The methodology adopted was a literature review, with a qualitative approach, aiming to understand the strategies used by healthcare professionals, especially the nursing team, in the transition from hospital to home care.The results highlighted the importance of the hospital discharge plan, health education, and caregiver involvement in the rehabilitation process. The essential role of nursing in guiding self-care, preventing complications, and providing emotional support to the patient and their family was emphasized. The need for integration between health services to ensure continuous and effective care was also observed. It is concluded that home care for post-stroke patients requires preparation of the healthcare team, support for caregivers, and public policies that ensure comprehensive rehabilitation and quality of life. It is recommended that future research investigate the effectiveness of home care programs and the perceptions of patients and caregivers regarding the challenges post-discharge.
