Background: Evaluate the effects of NAC in testes of rats subjected to ischemiareperfusion through torsion and detorsion of the spermatic chord. Methods: Forty-eight rats, divided into four groups with 12 animals each. The animals in groups I and II were subjected to twisting of the spermatic cord, and in group II received NAC. The animals in groups III and IV were exposed to the testicles and immersion in saline solution, and in group IV, received NAC. In groups I and II, the testes were distorted after 45 minutes of ischemia, and in all groups, the testes were repositioned in the scrotum after 60 minutes. After 60 days, the animals underwent bilateral orchiectomy with histopathological evaluation of the testes. Results: When comparing the right and left testicles in each group separately, there was statistically significant in histopathological degree changes in all groups. In assessing inter-groups when analyzing the right testicles, there was no statistically significant difference for groups III and IV, so the testes that did not undergo twisting of the spermatic cord, the drug showed no benefit. In the analysis of the right testicles in groups I and II, where there was torsion of the spermatic cord, the difference for GIII and GIV was significant (p <0.001). There was also difference between GI and GII (p <0.001), showing the protective effect of N-acetylcysteine in the testicles twisted (GII). To the left testicle was no statistical difference between groups I and II compared to groups III and IV (p = 0.023). There was no difference between GI and GII and between GIII and GIV. This shows that the torsion of the right testicle caused histological changes in contralateral testis, with no beneficial effect in the group receiving N-acetylcysteine (GI=GII>GIII=GIV), while groups without torsion, the histological changes were not significant. Conclusions: N-acetylcysteine showed a protective effect on cell morphology of the ipsilateral testes of rats after spermatic cord torsion followed by reperfusion, and was observed histological changes in contralateral testis.pt_BR
